تشخیص اختلال بویایی برای پیش‌بینی ابتلا به بیماری آلزایمر

بویایی یک سیستم حسی پیچیده است و علاقه به بررسی ارتباط بین بویایی و شناخت به ویژه در سال‌های اخیر رو به افزایش است. بویایی، توانایی‌های شناختی و حالت‌های فردی را تحت تأثیر قرار می‌دهد. کهولت سن و بیماری‌های اعصاب مرکزی به ویژه آلزایمر و پارکینسون، با ایجاد تغییرات آناتومی پاتولوژیکی ساختار‌های بویایی مرکزی و محیطی، باعث ایجاد اختلال در عملکرد بویایی می‌شوند. بنابراین یکی از اتفاقاتی که در افراد مبتلا به آلزایمر صورت می­‌گیرد، از دست دادن حس بویایی است.

در مطالعه‌ای که در سال 2014 روی افراد میانسال انجام شده‌است، رابطه­‌ی بین اختلال بویایی، ارزیابی­‌های شناختی و حجم ساختار مغز در افراد میانسال سالم در مقایسه با بیماران مبتلا به آسیب شناختی همراه با اختلالات فراموشی (aMCI) یا آلزایمر (AD) بررسی شد. 12 بیمار مبتلا به aMCI و نیز 6 بیمار با آلزایمر در مراحل اولیه و 30 نمونه­‌ی کنترل بررسی شدند. توانایی تشخیص بویایی به وسیله­‌ی تست تشخیص بویایی دانشگاه پنسیلوانیا (UPSIT) و تست sticks sniffin(تست بویایی ویژه‌­ی اروپا) ارزیابی شد و در کنار این تست­‌ها حجم ساختاری مغز با MRI  ارزیابی شد.

نتیجه­‌ی MRI بیماران مبتلا به آلزایمر که اختلال بویایی داشتند در مقایسه با بیمارانی که بویایی تقریباً سالم داشتند به طور قابل توجهی، بدتر بود. بین امتیازات تست بویایی و نتایج MRI در گروه بیماران، رابطه‌­ی معنی­‌داری وجود داشت.

نتایج این مطالعه نشان داد، کاهش اندازه­‌ی حجم‌­های مغزی مربوط به بیماری آلزایمر، به­ شدت در ارتباط با توانایی تشخیص بویایی است. بنابراین، نتایج ثابت می­‌کند اختلال بویایی، یک مارکر اولیه­‌ی تخریب لوب تمپورال مغز می‌باشد.

تحقیقات نشان می‌­دهند در افرادی که دچار آلزایمر می‌­شوند، قبل از ظاهر شدن هر علامت بالینی دیگری، حس بویایی دچار اختلال می­‌شود. بنابراین، با انجام تست­‌های ساده بویایی می­‌توان تا حدی پیش­‌بینی کرد که یک فرد میانسال در معرض ابتلا به آلزایمر هست یا نه. در واقع تست بویایی برای تشخیص آلزایمر، ارزان­‌ترین، کم­‌خطرترین و در دسترس­‌ترین وسیله برای تشخیص زودهنگام آلزایمر و کمک به بیمار برای مهار کردن پیشرفت بیماری است.
